I hereby grant permission for my daughter to participate in the Legacy Volleyball Camp and acknowledge that she is physically able to participate in camp activities. I release Mansfield Independent School District, the LHS Volleyball Camp, and employees from all claims due to injury or illness which may be sustained by my daughter at the LHS Volleyball Camp.
