Parent Authorization: I, as parent or guardian, hereby give permission for my child to participate in the Eagle Sports Camps and acknowledge the fact that he is physically able to participate in camp activities. I hereby authorize the directors and coaches of the Eagle Sports Camps to act for me in any emergency requiring medical attention. I also acknowledge that I will be responsible for any cost (through family medical insurance or otherwise) incurred due to the sickness or injury to my child. I hereby waive any claim that I might have against the Eagle Sports Camps and institution providing the facilities.
